Services can be billed as an Out-of-Pocket Network Provider with many insurance companies. It is your responsibility to determine if your particular insurance plan will allow this option. I can provide you with a detailed receipt, including appropriate billing codes and descriptions of services if needed. Payment in full is required at the time of service, even if you will be requesting reimbursement from your insurance.
